At your first appointment, our office staff will have you complete several patient information forms. For your convenience, the forms are accessible from our website (see below). It is best to call for prescriptions and refills between 9 a.m. and 4 p.m., weekdays.
This allows us to review your chart for allergies and accuracy. If an emergency occurs after hours make sure to have your medication name, dosage and the number of your pharmacy.
We can only prescribe medications to patients who are actively in our care.

Our physicians are providers for several insurance plans. Prior to your appointment each patient should check their insurance manual to insure their physician is a participating provider. Each patient should review their policy to be aware of rules and limitations for their plan.

Patients with urgent problems can usually be seen the same day. If you cannot make your appointment, please call to cancel at least 24 hours prior to your scheduled time so other patients can be accommodated.
There is a physician available at all times for emergencies. If you are calling for test results, appointments, or non-urgent problems, please call during our regular office hours.
